If you have concerns about your mental health or emotional wellbeing, a GP can discuss your symptoms, complete an assessment and recommend appropriate support or treatment options. Where clinically appropriate, this may include preparing a Mental Health Treatment Plan.

why should i have a mental health assessment?

A mental health assessment can help your GP understand your symptoms, concerns and individual circumstances. Your GP can then discuss suitable support, treatment or referral options with you.

Having a mental health assessment gives your doctor a picture of the way you think, feel, reason and remember. You will be required to answer a series of questions to assess your emotional wellbeing – and may also include a physical examination.T

how can a mental health assessment help me?

A Mental Health Assessment is designed to help your Doctor diagnose where you are at in the following areas and then move forward in preparing a mental health plan from there. Depending on your individual circumstances, an assessment may help your doctor to:

  • diagnose mental health conditions such as anxiety, depression, schizophrenia, postnatal depression, eating disorders and psychotic illnesses  
  • differentiate between mental and physical health problems
  • assess a person referred because of problems at school, work or home.

how do i prepare for a mental health assessment?

Before your appointment, you may be ask to think about the reasons for the assessment:

  • What behaviours, thoughts or feelings are troubling you and how often you experience them and what were you doing when you have them
  • Was there a particular event which brought on symptoms; such as a death of a loved one?
  • How long do symptoms last?
  • You may be asked to keep a diary and list any prescriptions or drugs you are taking – some drugs can attribute to the way you think or feel, which may explain some of your symptoms.

At your appointment:

A mental health assessment is includes a few questions followed by a physical examination, along with a questionnaire either written or verbal – these tests are designed to assess:

  • How well you can think, reason and remember
  • Specific problems such as depression or anxiety
  • How well you carry out daily life activities such as dressing, eating, shopping.


Following your assessment, your GP may recommend or provide a referral to another healthcare professional, such as a psychologist or psychiatrist, where clinically appropriate.
