Why do we Collect, Use, Hold and Share Your Personal Information?

Westridge Medical will need to collect your personal information to provide healthcare services to you. Our main purpose for collecting, using, holding and sharing your personal information is to manage your health. We also use it for directly related business activities, such as financial claims and payments, practice audits and accreditation, and business processes (eg staff training).

Our patients have the right to request to access and correct their personal health information under the Privacy Amendment (Private Sector) Act 2000. This is a written request for access to your personal information, and the Practice has 30 days to respond to this request.

Please contact our Reception team should you wish to access your personal information, and they will provide you with the written request form to complete.

All third party requests for personal health information must contain a signed consent from the patient before these are released by the treating GP.
